Course Description: This course is an extension of Care Coordination as a Model of Care to explore in more detail the purpose of a multidisciplinary team and how multidisciplinary teams better support patient outcomes.

To pass this course you must view the lecture, complete the pre-test and evaluation and score at least 80% on the post-test.
