Course Description: This program will help healthcare professionals conduct effective transition planning. Participants will learn how to assess when a transition to a higher level of care is necessary and become familiar with the tools and resources available to assist in this process.
Instructional Method: Asynchronous online PowerPoint® lecture, Pre-Test, Post-Test and Evaluation
To pass this course you must view the lecture, complete the pre-test and evaluation and score at least 80% on the post-test.

Target Audience: Nursing, Social Workers, Medical Assistants, and other interested healthcare providers
At the end of this presentation, the learner will be able to:
Identify the needs of aging Americans
Discuss perspectives on the resources available to older adults
Collect information on how to appropriately assess seniors for transitioning to a higher level of care
